The failure this process exists to prevent is the quiet close.

A complaint that went away. No decision anybody wrote down, no record of who considered it, nothing that can be shown afterwards to have been done properly — which fails the person who complained, the person complained about, and the company, in that order.

What cannot be done

The guarantees here are refusals.

An inquiry cannot open unless the committee is properly composed — checked at the moment the inquiry starts, not asserted in a policy document. A second presiding officer cannot be appointed. A decision cannot be recorded without the reasoning written down, and a case cannot be withdrawn or dismissed without a stated reason.

Nothing in the record can be edited or removed. The ledger only grows, and a closed case stops accepting entries entirely.

The record

When it happened, and when it was written down.

Every entry carries the date the thing actually happened and the moment it was entered, by whom. So a hearing logged a week late is recorded honestly as a hearing held then and entered now, rather than quietly back-dated.

Where the person complained about is an employee, they are chosen from the directory and the record takes their canonical name — so a register cannot drift into two spellings of one person, which is exactly what makes a pattern across years invisible.

  • Filing can be anonymous

    Submitted from the portal without the complainant's identity attached. What an inquiry can do with an anonymous complaint is genuinely more limited, and that constraint is real rather than papered over.

  • Dismissal at first look is a recorded act

    A complaint that does not proceed is closed explicitly with that reason, rather than going quiet.

  • Withdrawal is available and recorded

    A complainant is allowed to change their mind, and that decision is theirs and documented.

  • The annual report comes out of the register

    Including the count still open past the statutory age — computed as at the year end, so re-running a past year gives the same answer rather than today's.

What we are not claiming

Read this section before anything else on the page.

Can somebody file without signing in?

No. Filing is from the employee portal — identified or anonymous, but signed in either way. Someone who has already left, or a contractor without an account, cannot file through this. That is a real gap and it matters for exactly the people most likely to need it.

Can the complainant follow the case?

They can see that their case exists and its state. They do not see the committee's record of actions — there is no complainant-facing view of that today. Keeping somebody informed is currently a person's job, not the product's.

Who can read a case?

Anyone holding the committee permission. It is a single gate, not a per-case membership check, so scope it narrowly and deliberately — this is the most sensitive data in the product and the access model is blunter than it should be.

Does this make us compliant?

No. Compliance is a properly constituted committee, trained people and an inquiry conducted correctly. The product holds the register, the process and the record — necessary, nowhere near sufficient.

Does anything assess the complaint?

No. Nothing scores, classifies or triages. The judgement belongs entirely to the committee.

Can a complaint be deleted?

No. Cases are closed, withdrawn or dismissed with a reason. A register that can be emptied is not a register.
